• Medical Records Transfer to DCDA – Please print, complete, and return to us at DCDA. This will allow us to obtain your existing medical records from your doctor.

Transfer Medical Records

• Medical Records Transfer from DCDA To Another Doctor – Please print, complete, and return to us at DCDA to authorize us to release your medical records to another doctor.
• Medical Records Transfer to DCDA – Please print, complete, and return to us at DCDA. This will allow us to obtain your existing medical records from your doctor.
